I posted a comment for one article that might fit number 2. I've been accused by my writers club of something similar, called an "Intuitively Obvious" moment. I had no clue what I just read in your article. Here was my comment: "...It should, at least, include clear definitions for each term." That had nothing to do with English or grammar. Even a footnote at the bottom would have worked. Of course, the club insisted that I add the traits of a cat to another article where I said I was like one.
