The first problem is that people don’t know how to estimate time.

First rule of project management: Estimate how long something will take, then double it. Assume that everything will take twice as long as you think. That way, you don’t try to do too many things at the same time.

Second, don’t make the same mistake twice.

You hit the third problem perfectly — people lose focus.
