I’ll give you an example to support one of your points. I spent 5 months in Peru. They have the government clinic and private HMOs for those who could afford them — $300 per year + copay. When I was growing up, we had something similar. We had the “County Clinic” for the poor and private health care. That has disappeared; I think because it was not “equal.”
I suspect that many of the problems you pointed out are partly due to a credit-based society keeping people in debt.
